Registered Nurse (RN) - Skilled Nursing Facility (SNF)
Location: Schofield, WI
Position Type: Contract / Travel Assignment
Shift: 4x8 Mid Shift
Assignment Duration: 14 Weeks
Expected Start Date: 8/19/2026
Weekly Compensation: Up to $2,321.71 / week
Position Overview
We are seeking a compassionate and skilled Registered Nurse (RN) for a 14-week travel assignment at a highly respected long-term care and rehabilitation facility in Schofield, Wisconsin. The ideal candidate will have strong clinical expertise in skilled nursing or sub-acute settings, exceptional patient care skills, and the ability to adapt seamlessly to floating across diverse clinical units.
Key Responsibilities
- Provide comprehensive nursing care to short-term rehabilitation, long-term care (LTC), and sub-acute patients.
- Administer medications and perform treatments, including wound care, stoma care, wound vacs, Foley catheter management, and tube feedings (Peg, J-tube, G-tube).
- Perform basic tracheostomy care as needed.
- Start IVs, administer IV fluids/antibiotics, and perform blood draws.
- Monitor, record, and report patient condition changes using the facility's Electronic Medical Record (EMR) system (PointClickCare).
- Float as needed across all facility units (Rehab, SNF, LTAC, LTC, and Short-Term Stay).
Qualifications & Requirements
- Experience: Minimum of 2 years of nursing experience in a Skilled Nursing Facility (SNF), Long-Term Care (LTC), LTAC, or Rehab setting required.
- Licensure: Active Registered Nurse (RN) license in the state of Wisconsin (or compact RN license) with clean background and clear license history.
- Certifications:
- Active BLS/CPR certification required.
- IV certification preferred (or demonstrated proficiency in IV insertion and lab draws).
- Location Restriction: Candidates must reside at least 50 minutes outside of the facility location.
